The presenter took a tumble when she slipped on her patio tiles during a downpour.
The fall was captured on CCTV so Hammond shared it on the ITV show – where she also revealed that she hadn’t been wearing any underwear when she went down.
The clip showed the star walking across the lawn then stepping onto a tiled path and falling over.
A heart was imposed over the footage and Hammond told co-host Dermot O’Leary and viewers that it was to protect her modesty.
"The reason why there is a little heart there is because when I go home I like to chill out and I like to take all my underwear off, so there's a little heart there to just cover my modesty," she laughed.
"It was quite a bad fall you know,” she went on, as O’Leary and crew members could be heard giggling.
"You’ve got to imagine, it's raining, it's full on... I can't get up because I think I've broken my leg.
"My brother comes out to try and he’s trying to lift me up, his bottom is showing all on the CCTV. He can't lift me up because obviously I'm quite big.
"He helps me up, I eventually use the bad leg and then I totter inside."
Hammond said her leg looked "absolutely awful" and wanted to show viewers a picture, but O’Leary joked that it was "too much pre-watershed".
The presenter settled for lifting the hem of her dress to show the bandage instead.
Hammond then shared that when she cut herself she noticed a vein on the back of her leg and was worried it was a deep vein thrombosis (DVT) so went to hospital.
She was there for seven hours, but luckily it was a false alarm.
